Facilitator Certification Training Program Registration Form 

Register for TWMFR Facilitator Training!

If you’ve come this far, you’d like to learn more about this program or you are good to go and want to sign up!  There are some important items to note before you reach out to us about the program:

  • Open to residents of Alberta only
  • You must have previous First Responder experience (2 years minimum)
  • You will be expected to provide a Letter of Support from your organization or a reference if you are applying as an individual 
  • You are required to attend.  Cancellation may be subject to the full amount of the program ($3,000) becoming immediately due.  As we only have 48 spots available, we need to maximize the training provided by the publicly funded grant dollars

Important Note

This training is NOT offered on a “first come first serve” basis.  As there are limited seats available, we wish to provide maximum impact for our grant dollars.  Please submit your registration to get on the list.  We will advise you as the registration and application process proceeds.

Registration Process

Step 1 - Fill Out and Submit the Registration Form

Step 2 - Application Process

Once we have received your Registration, you will receive 4 documents from us:

TWMFR FCT Application Form

This is the main application form for the TWMFR and Family Package Facilitator Certification Training (FCT) from Opening Minds (OM) / Mental Health Commission of Canada (MHCC).

AMHSA SPHIFR Participant Agreement

This form from the Alberta Municipal Health and Safety Association (AMHSA) contains the commitment from you to attend the Faciliator Certification Training (FCT), provide a backup resource in case you cannot attend and fill out the pre and post survey forms.  As this is a limited seat event, cancellation will have you be subject to paying for the full course fees.

TWMFR Letter of Support instructions

You will need to provide references for support into this program.

If you are part of an organization, this letter must include specific reference to how the organizaton will support the applicant in the following points.  If you are an independent faciliator, you will have to provide two reference letters.

Letter of Support details:

  • Providing time to prepare and deliver at least 2 The Working Mind First Responders (TWMFR) courses per facilitator year to maintain active status.
  • Providing space and/or an online platorm to deliver TWMFR courses.
  • Providing time for applicants to atend mandatory webinars, meetngs, etc.

In addition, the letter must address on how the applicant meets the criteria to become a Facilitator:

  • Experience supporting mental health and well-being at work or in the community
  • Ability to lead group discussions for groups of 8 or more.
  • Ability to facilitate a minimum number of courses on a yearly basis (as outlined in the “Facilitator Contract) and a commitment to meeting this requirement.
  • Positive attitudes towards people living with mental health problems
  • Driven to reduce stigma/discrimination associated with mental health in the workplace
  • Proficient on PowerPoint
  • If applicable: Knowledge of virtual delivery platforms such as Zoom or MS Teams. The virtual FCT is exclusively delivered on the Zoom platform.

TWMFR Facilitator Agreement (for informational purposes)

This agreement will by signed by you after the training is completed to acknowledge your commitment to the program and the minimum requirements for maintaining your facilitator Status

Step 3 - Applicant Review

AMHSA will review applications and requested timing to produce a list of applicants to send forward to OM / MHCC to complete the application process.

OM / MHCC will review all supplied application documentation to approve applicants and will respond to AMHSA with list of approved applications within 2 weeks.

Please note: Your registration and application does not guarantee a spot in the training.  We will be processing applications based on providing the maximum impact for the grant dollars.

Step 4 - Applicant Notification and Confirmation

OM / MHCC will respond to the approved Applicants to notify them if they are approved and confirm that they are still able to attend the training for the selected dates.  Applicants will be given one calendar week to respond. 

Upon confirmation from the applicant, AMHSA will notify OM / MHCC and start the booking process. 

Step 5 - Training Setup

The following action will be performed by OM / MHCC starting 4 weeks from the training start date.

Cut-off date to implements backups/ substitutions
